First & foremost, we would like to THANK YOU for joining us to make this Community Health & Wellness Fair a huge success! We believe this event will be a tremendous benefit to the Community, as well as for your business to showcase your health & wellness products. Follow the steps below to complete your Registration.

  • Please review each of the Vendor Levels in the flyer below. You can click on the flyer to enlarge or download a PDF.

  • Proceed to the Registration Form below.

  • There are 2 steps to complete your Registration. You must complete BOTH steps to be completely registered for this event. Contact Sherri Kelley (561-373-2802) or Rosemarie Glade (352-345-7238) if you have questions.

    • 1. Fill in the Registration Form and click Submit.

    • 2. Click the "Payment" Button to complete your Payment. *(Payments are made directly to SmileFaith Foundation.)

Vendor & Sponsorship information. Level 1 is $150. Level @ is $275. Level 3 is $500.


Feel free to have your own raffle at your table! This will get you more contacts and exposure.
